When is icsi used instead of ivf

Eggs must have all of the protective granulosa cells removed prior to ICSI so that the polar body can be visualized. This process was initially developed for men whose sperm had low motility , and could not penetrate the zona pellucida. The process that leads up to the insemination of eggs with sperm is called controlled ovarian hyperstimulation.

Once the eggs have been retrieved during an in-office procedure called an egg oocyte retrieval, it is time for the true IVF to begin. Eggs can be fertilized via ICSI or conventional insemination. Conventional IVF was developed to circumvent female fertility problems such as blockages in the Fallopian tubes, endometriosis, or when previous IUI cycles were not successful.

If using conventional fertilization, eggs are placed in media drops and sperm is then added to those drops. ICSI is a more recent technology developed with male factor infertility in mind, allowing men with low sperm count oligozoospermia or immotile sperm asthenozoospermia to father a child. During traditional IVF, many sperm are placed together with an egg in hopes that one of the sperm will enter and fertilize the egg on its own.

With ICSI, the embryologist takes a single sperm and injects it directly into an egg. But fertilization rates are different than clinical pregnancy or live birth rates. ICSI has the ability to achieve good reproductive outcomes in cases where traditional IVF would not, however in cases where male infertility is not an issue, multiple studies have demonstrated fertilization and pregnancy success rates with ICSI are equivalent to standard insemination by IVF. The success rates of each method are highly dependent on the skill of the embryologist, egg and sperm quality, and the overall health and causes of infertility in both partners.

All things being equal, one method is not better than the other. If there are a limited number of eggs, ICSI often preferred because it optimizes the odds as the highest quality sperm are selected. When donor sperm or eggs are used, ICSI is often considered unnecessary because the quality of both gametes is so high. During regular IVF, many sperm are placed together with an egg, in hopes that one of the sperm will enter and fertilize the egg on its own.

Others reserve the treatment for those with severe male infertility or another medically indicated reason. With that said, ICSI-IVF has enabled many infertile couples to get pregnant when, without it, they would not have been able to conceive using their own eggs and sperm. If a man does not have any sperm in his ejaculate, but he is producing sperm, they may be retrieved through testicular sperm extraction or TESE.

ICSI is also used in cases of retrograde ejaculation if the sperm are retrieved from the man's urine. As with regular IVF, you'll take ovarian stimulating drugs , and your doctor will monitor your progress with blood tests and ultrasounds. Once you've grown enough good-sized follicles , you'll have the egg retrieval , where eggs are removed from your ovaries with a specialized, ultrasound-guided needle.

Your partner will provide their sperm sample that same day unless you're using a sperm donor or previously frozen sperm. Once the eggs are retrieved, an embryologist will place the eggs in a special culture and using a microscope and tiny needle, a single sperm will be injected into an egg. This will be done for each egg retrieved. If fertilization takes place, and the embryos are healthy, an embryo or two will be transferred to your uterus, via a catheter placed through the cervix, two to five days after the retrieval.

It may cost more than this if other IVF options are being used. A normal pregnancy comes with a 1. ICSI treatment carries a slightly increased risk of birth defects, but it's still rare.

There is also a slightly increased risk of a male baby having fertility problems in the future. This is because male infertility may be passed on genetically. It's one thing if you need ICSI to conceive. Then, you can discuss with your doctors the pros and cons of using this assisted reproduction technology. The ICSI procedure fertilizes 50 to 80 percent of eggs. When is ICSI used? Share this. Request an appointment General enquiry.
